Below is a listing of the forms that are to be completed and returned to the Council office.
The green buttons are the forms that need to be received at the Council office in order to qualify for the additional 2% commission. The prizes are to be entered online by the unit popcorn chair, but the back page with the Walmart gift card information must be received at the Council office before any gift cards can be released.
